By clicking the "submit" button below, I submit this order designating JEFTINI POZIVI, a Corporation with its principal place of business In Illinois to switch my provider(s) for long distance service from my current telecommunications carrier(s) for each of the telephone numbers or facilities listed below. I authorize JEFTINI POZIVI and its network(s) providers to gather information (on an individual or on-going basis) on telecommunications services provided. This information shall include, without limitation, all previous and current invoices, customer service records, technical configuration, correspondence,contractual information, contract copies, tariffs and special arrangements provided by all telecommunications service of Interstate, and International Long Distance.I also understand that I may designate only one interLATA carrier per telephone, and that this change may involve a charge by the local exchange carrier. I understand that I may consult JEFTINI POZIVI as to whether a fee applies to change from my current telecommunications carrier (s). In some case charges may apply for switching services by my existing carrier. Further, I understand that the monthly rate plan does not include taxes or applicable regulatory fees.I choose JEFTINI POZIVI to effectuate the above change(s) and authorize JEFTINI POZIVI and its subcontractors to handle on my behalf all arrangements, including ordering,redirecting invoices on an individual or on-going basis, changing, and/or maintaining any new or existing service(s), with my local telephone company(s), interexchange carriers, joint user group(s), equipment vendor(s)and consultant(s).In addition, I authorize JEFTINI POZIVI to issue all necessary instructions to the foregoing for the purpose of fulfilling its agency on my behalf. This agreement will remain in effect until revoked in writing by you.I represent and warrant that I am the party identified in the account records of my local telephone company (or other provider) as responsible for payment and that I have the authority to make decisions for the telecommunications services identified herein including the authority to change telecommunications carriers, and appoint JEFTINI POZIVI as agent to act in accordance with this Letter of Authorization.I agree to indemnify and hold JEFTINI POZIVI harmless for all liability and expenses for any breach of that representation and warranty.
